HCW Biologics Inc. (NASDAQ: HCWB) reported second-quarter 2026 business and clinical development updates, highlighting continued advancement of its HCW9302 fusion immunotherapeutic in alopecia areata. Preliminary human data from the first two cohorts of the Phase 1 study support the potential of HCW9302 to activate and expand regulatory T cells (Tregs) while limiting broader immune activation. In the second dose cohort, all three participants receiving a single 3 micrograms/kg subcutaneous dose showed preliminary improvement in Severity of Alopecia Tool (SALT) scores, with each demonstrating at least a 25% reduction from baseline at four and/or nine weeks after treatment. Importantly, the company reported no dose-limiting toxicities, while no cases of capillary leak syndrome or cytokine release syndrome were observed. HCW Biologics remains on track to provide a full Phase 1 data readout in the fourth quarter of 2026, with treatment underway in the third dose cohort at 8 micrograms/kg.
HCW9302 Shows Early Clinical Activity in Alopecia Areata
The emerging clinical profile of HCW9302 represents the central development focus of HCW Biologics’ autoimmune disease program. The investigational therapy is designed as an IL-2-based fusion immunotherapeutic intended to preferentially stimulate Treg cells, which play an important role in controlling abnormal immune responses. The company believes this mechanism could potentially reduce inflammation without causing the broad immunosuppression associated with some existing autoimmune therapies. The early Phase 1 findings are particularly notable because HCW9302 is being evaluated as a monotherapy at relatively low doses, rather than being combined with another immunomodulatory treatment. The company also reported that treatment did not increase blood eosinophil counts, another concern associated with certain IL-2 therapies. While the dataset remains very small and preliminary, the observed SALT improvements provide an early clinical signal that supports continued development. T-Cell Engager Program Moves Toward IND Planning
Beyond HCW9302, HCW Biologics is advancing its HCW11-018b T-cell engager (TCE) program toward clinical development in cancer. The company has requested a Type B pre-IND meeting with the FDA to discuss the development and regulatory strategy for HCW11-018b and to seek agreement on requirements for a future clinical study before submitting an IND. The company expects the planned clinical trial to potentially begin in the first half of 2027, subject to FDA authorization. HCW11-018b is a tetravalent T-cell engager designed for subcutaneous administration and is being developed for solid tumors. Preclinical studies have indicated activity against tissue factor-expressing cancer cells while activating CD3-positive effector T cells and addressing immunosuppressive features of the tumor microenvironment. The program is supported by HCW Biologics’ proprietary TRBC drug development platform and a manufacturing approach based on recombinant CHO cell lines. These developments add a second important clinical-development pathway alongside HCW9302 and expand the company’s focus across autoimmune disease and oncology..

HCW Biologics also continued efforts to develop commercial opportunities around its HCW9206 commercial-ready reagent molecules, particularly for CAR-T cell manufacturing. Following the termination of its suspended AlloTera Therapeutics license, the company re-acquired ex vivo rights to two molecules and is pursuing a corporate partner to support their commercialization. Research conducted with the Albert Einstein College of Medicine and published in Science Advances indicated that HCW9206 could improve CAR-T manufacturing characteristics, including long-term persistence and functionality of T memory stem cells. Financially, the company reported $6.7 million in revenue for the first half of 2026, while second-quarter R&D expenses remained approximately $1.2 million. HCW Biologics also raised approximately $5.6 million through equity financings during May and July. However, the company disclosed substantial doubt regarding its ability to continue as a going concern without additional funding or financial support, making financing and business-development execution important considerations as clinical programs advance. For the immediate pipeline, the most closely watched milestones remain the HCW9302 Phase 1 data readout in Q4 2026 and regulatory progress toward the planned HCW11-018b clinical study.
