UpDoc has been selected by the Advanced Research Projects Agency for Health (ARPA-H) as a lead performer in the Agentic AI-Enabled Cardiovascular Care Transformation (ADVOCATE) program, receiving an award of up to $9.2 million to develop and validate an autonomous clinical artificial intelligence system. The UpDoc-led initiative is supported by Microsoft, OpenAI and NVIDIA and is designed to advance a future FDA-authorized clinical AI system capable of providing continuous cardiovascular care. The program will initially focus on heart failure, including management of medications, diet and exercise.
UpDoc Advances Autonomous Clinical AI
The ARPA-H initiative represents a significant step in the development of agentic clinical AI, with UpDoc tasked with directing the development of technology intended to extend proactive and personalized care to patients who may face limited access to specialty services. The program aims to develop clinical AI agents capable of supporting complex care activities while operating within appropriate clinical safeguards, regulatory requirements and physician oversight. The ADVOCATE program is structured as a 39-month, multi-phase development effort. Participating organizations must advance through competitive milestone evaluations to progress toward later stages of the program. The planned pathway includes independent verification and validation, performance benchmarking against cardiologists, and clinical trials conducted under FDA Investigational Device Exemption (IDE) requirements. Large-scale studies within partner health systems will also evaluate AI-enabled care against usual care while incorporating electronic health record (EHR) integration. According to ARPA-H, the program is intended to establish a pathway from prototype development through clinical evidence generation and potential real-world deployment. The initiative is notable because it is designed from the outset to address the technical, regulatory, reimbursement and deployment requirements associated with advanced clinical AI.
FDA-Cleared UpDoc Platform Supports Care Delivery
UpDoc already has an FDA-cleared clinical AI platform that combines medical device functionality with conversational artificial intelligence. Its current cleared system supports insulin management within physician-prescribed treatment plans and defined clinical boundaries. The platform is integrated with provider workflows and is designed to allow clinicians to extend care beyond traditional scheduled appointments. The company describes its technology as an enterprise-ready clinical AI system designed to connect with electronic health records and clinical workflows. Its proprietary Clinical Intelligence System brings together patient information, clinical guidelines, physician governance and safety controls to support real-world care delivery. The next-generation system being developed through ADVOCATE is intended to go beyond the company’s existing capabilities. UpDoc plans to develop an agentic architecture in which advanced large language models can reason over changing patient information and select appropriate diagnostic, treatment and care-management actions. These activities would remain within UpDoc’s clinical intelligence and governance framework, with safeguards intended to support auditability, accountability and physician oversight. Importantly, the company stated that this next evolution of UpDoc’s technology has not been cleared by the FDA. Its future regulatory status will depend on development, validation and regulatory review.
UpDoc Targets Continuous Cardiovascular Care
The initial ADVOCATE focus is cardiovascular disease, with the program specifically targeting autonomous management of heart failure. The proposed clinical AI system is intended to operate as a digital member of the care team, supporting patients continuously rather than limiting intervention to scheduled healthcare encounters. UpDoc’s platform is already undergoing deployment at healthcare organizations including Cleveland Clinic, Allegheny Health Network and UCSF Health, providing an early real-world environment for its clinical AI technology. The company’s existing system is designed to help care teams provide personalized interventions between visits while maintaining clinician involvement. Through the ADVOCATE program, UpDoc will work with ARPA-H, FDA and other healthcare stakeholders to address the challenges associated with bringing autonomous clinical AI into real-world medical care. The initiative is expected to generate evidence around system performance, safety, scalability and clinical effectiveness. The collaboration marks an important development in AI-enabled healthcare delivery, as autonomous clinical systems move from experimental concepts toward structured regulatory and clinical evaluation. If UpDoc successfully progresses through the program’s milestones, the resulting evidence could help establish a framework for deploying AI-supported cardiovascular care at scale while maintaining clinical governance and patient safety.
