BeOne Medicines has announced positive topline results from the Phase 3 MANGROVE clinical trial, demonstrating that BRUKINSA® (zanubrutinib) combined with rituximab significantly improved outcomes for patients with previously untreated mantle cell lymphoma (MCL). The landmark study achieved its primary endpoint by reducing the risk of disease progression or death by 43% compared with the current standard bendamustine plus rituximab chemotherapy regimen. The findings position BRUKINSA as a potential new chemotherapy-free frontline treatment for one of the most aggressive forms of B-cell non-Hodgkin lymphoma, offering patients the possibility of effective disease control while avoiding the toxicities and long-term burden associated with conventional chemotherapy. The company plans to present full efficacy and safety data at an upcoming scientific meeting and expects to submit global regulatory applications during the second half of 2026, potentially establishing a new standard of care for frontline mantle cell lymphoma.
Phase 3 MANGROVE Trial Meets Primary Endpoint
The MANGROVE study is the first global Phase 3 randomized trial to compare a chemotherapy-free Bruton tyrosine kinase (BTK) inhibitor-based regimen directly against standard chemoimmunotherapy in newly diagnosed mantle cell lymphoma. The trial enrolled 510 adult patients across 176 clinical sites worldwide, evaluating BRUKINSA plus rituximab versus bendamustine plus rituximab. At the pre-specified interim analysis, the investigational regimen demonstrated a 43% reduction in the risk of disease progression or death (Hazard Ratio 0.57; p<0.0001) based on independent review committee assessment, successfully meeting the study’s primary endpoint of progression-free survival (PFS).
Importantly, the treatment eliminated the need for maintenance rituximab and prolonged chemotherapy, potentially sparing patients nearly two years of infusion-based therapy while maintaining superior disease control. Overall survival data remain immature, although investigators reported a strong positive trend favoring the BRUKINSA regimen. The safety profile remained consistent with previously established clinical experience, with no new safety signals identified during the study.
Chemotherapy-Free Strategy Addresses Unmet Clinical Need
Mantle cell lymphoma is a rare but highly aggressive subtype of B-cell non-Hodgkin lymphoma that primarily affects older adults, many of whom have additional medical conditions limiting their ability to tolerate intensive chemotherapy. Although bendamustine plus rituximab has remained a frontline standard of care for many years, chemotherapy is frequently associated with bone marrow suppression, prolonged immune dysfunction, increased infection risk, and cumulative toxicity. The MANGROVE trial represents a significant shift in treatment strategy by replacing chemotherapy entirely with a targeted BTK inhibitor while preserving clinical efficacy.
BRUKINSA is a next-generation BTK inhibitor engineered to provide sustained and selective inhibition of Bruton tyrosine kinase through optimized pharmacokinetics, enabling consistent disease control across multiple B-cell malignancies.
