WAYNE, Pennsylvania, October 28, 2026
Palvella Therapeutics, Inc. has announced that the U.S. Food and Drug Administration (FDA) has accepted its New Drug Application (NDA) for QTORIN™ 3.9% rapamycin anhydrous gel and granted the application Priority Review for the treatment of microcystic lymphatic malformations (microcystic LMs). The FDA has assigned a Prescription Drug User Fee Act (PDUFA) target action date of February 28, 2027, marking an important regulatory milestone for the company and its efforts to develop treatments for rare skin diseases. If approved, QTORIN™ rapamycin could become the first FDA-approved treatment specifically for microcystic lymphatic malformations, a serious and lifelong condition affecting more than 30,000 diagnosed children and adults in the United States. The regulatory development represents progress toward addressing an unmet medical need among patients who currently have no FDA-approved treatment options for this condition.
FDA Grants Priority Review for QTORIN Rapamycin NDA
The FDA’s acceptance of Palvella’s NDA confirms that the application has been accepted for filing and will undergo an expedited regulatory review under the Priority Review programme. The February 28, 2027 PDUFA target action date establishes the anticipated date by which the agency aims to complete its review and communicate its regulatory decision. The application is supported by clinical evidence, including results from the company’s Phase 3 SELVA clinical trial, Phase 2 clinical findings, published scientific literature and additional evidence relating to the clinical use of off-label rapamycin in microcystic lymphatic malformations. QTORIN™ rapamycin has also received FDA Breakthrough Therapy, Fast Track and Orphan Drug designations, reflecting the regulatory attention given to its development for this rare condition. These designations are intended to support the development and review of potential therapies addressing serious diseases and significant unmet medical needs. However, Priority Review does not guarantee regulatory approval, and the FDA will continue evaluating the application’s clinical and other supporting evidence.
Phase 3 SELVA Trial Demonstrates Significant Clinical Results
A major component of the NDA submission is the Phase 3 SELVA study, which evaluated QTORIN™ rapamycin in patients with microcystic lymphatic malformations. According to Palvella, the study achieved highly statistically significant results across all six prespecified efficacy endpoints, with all reported p-values below 0.001. Furthermore, 95% of participants aged six years and older who completed the efficacy evaluation period demonstrated improvement on the microcystic lymphatic malformation Investigator Global Assessment (mLM-IGA) primary endpoint. These findings provide important clinical evidence supporting the company’s regulatory submission and highlight the potential of topical rapamycin treatment for patients living with this rare disease. The company also reported that QTORIN™ rapamycin was generally well tolerated in the Phase 3 SELVA study. No drug-related serious adverse events were reported, and systemic rapamycin concentrations remained below 2 ng/mL at all measured timepoints for all participants. These safety observations form part of the clinical evidence being considered during the FDA review.
Addressing Unmet Needs in Rare Lymphatic Disorders
Microcystic lymphatic malformations are rare, chronically debilitating conditions caused by abnormalities in the development of lymphatic vessels and associated dysregulation of the PI3K/mTOR signalling pathway. The condition can cause abnormal lymphatic vessels to develop within the skin and surrounding tissues, resulting in persistent fluid leakage, bleeding, recurrent infections, cellulitis and functional impairment. Patients may experience substantial physical discomfort and require repeated medical interventions throughout their lives. Existing treatment approaches, including surgery, sclerotherapy and laser procedures, can have limitations, including recurrence and the need for additional procedures. Systemic PI3K/mTOR inhibitors may also present challenges involving long-term safety and tolerability, particularly in paediatric patients. Palvella’s QTORIN™ platform is designed to support the development of topical treatments for serious rare skin diseases and vascular anomalies. Beyond microcystic lymphatic malformations, the company is also investigating QTORIN™ rapamycin for cutaneous venous malformations and clinically significant angiokeratomas. Its pipeline additionally includes QTORIN™ pitavastatin for disseminated superficial actinic porokeratosis.
Following the FDA’s acceptance of the NDA, Palvella Therapeutics is preparing for a potential standalone commercial launch in the United States during the first half of 2027, subject to regulatory approval. The company has established commercial leadership, medical affairs capabilities, a field-based medical science liaison team and dedicated patient services infrastructure to support potential product availability. Its preparations include disease education, engagement with healthcare specialists, patient identification and market access planning. If approved, QTORIN™ rapamycin could represent a significant development in the treatment landscape for rare lymphatic disorders and expand targeted therapeutic options for affected patients. The FDA’s final decision, expected by February 28, 2027, will be an important milestone in determining whether this investigational treatment can reach patients in the United States.
