Lighthouse Pharmaceuticals has announced new clinical findings from its Phase 2/3 GAIN trial, demonstrating that atuzaginstat, an investigational gingipain inhibitor, significantly improved neuropsychiatric symptoms and reduced caregiver distress in Porphyromonas gingivalis (P. gingivalis)-positive Alzheimer’s disease patients. The data, presented at the Alzheimer’s Association International Conference (AAIC) 2026, further strengthen growing evidence supporting the role of targeting bacterial infection and neuroinflammation as a potential therapeutic strategy for Alzheimer’s disease. Building upon previously reported clinical results showing that atuzaginstat slowed cognitive decline by 57% on the ADAS-Cog11 scale in a predefined subgroup of P. gingivalis-positive patients, the latest analyses demonstrate meaningful improvements in behavioral symptoms that greatly affect patients’ quality of life and caregiver burden. These findings represent an important advancement in precision medicine approaches for Alzheimer’s disease by identifying patients most likely to benefit through biomarker-guided treatment.
GAIN Trial Shows Improvements in Behavioral Symptoms
The latest analyses focused on Neuropsychiatric Inventory (NPI) subdomains among patients treated with atuzaginstat over 48 weeks. Investigators reported statistically significant improvements in agitation, aggression, disinhibition, apathy, and elation/euphoria compared with placebo among P. gingivalis-positive participants. These behavioral improvements were accompanied by a significant reduction in caregiver distress, suggesting clinically meaningful benefits extending beyond cognitive function. Importantly, investigators observed no comparable benefit in P. gingivalis-negative patients, reinforcing the importance of biomarker-guided patient selection and supporting the hypothesis that gingipain inhibition specifically targets disease mechanisms associated with bacterial infection in Alzheimer’s disease. The results further validate the growing scientific interest in the relationship between oral pathogens, chronic inflammation, and neurodegeneration.
Next-Generation Gingipain Inhibitor Advances Clinical Development
Building on these encouraging findings, Lighthouse Pharmaceuticals continues advancing LHP588, its next-generation once-daily gingipain inhibitor, which has already demonstrated a favorable safety profile during Phase 1 clinical evaluation. The company is currently enrolling approximately 300 P. gingivalis-positive Alzheimer’s disease patients into the ongoing Phase 2 SPRING clinical trial across multiple sites in the United States. The study is designed to evaluate the impact of LHP588 on cognition, agitation, aggression, daily functioning, and overall disease progression while further validating gingipain inhibition as a disease-modifying therapeutic strategy. The SPRING trial is supported by the National Institute on Aging, highlighting growing institutional interest in innovative treatment approaches targeting previously underexplored biological pathways in Alzheimer’s disease.
