Mumbai, India, and Princeton, New Jersey, United States, October 20, 2026
Sun Pharmaceutical Industries Limited (Sun Pharma) has announced that UNLOXCYT® (cosibelimab-ipdl) has received a Category 2A preferred recommendation in the updated National Comprehensive Cancer Network (NCCN) Clinical Practice Guidelines for patients with locally advanced cutaneous squamous cell carcinoma (laCSCC) who are not candidates for curative surgery or curative radiation. Announced on October 20, 2026, the guideline update represents an important development in the treatment landscape for advanced skin cancer. UNLOXCYT is an immunotherapy that blocks programmed death-ligand 1 (PD-L1), a protein that can help cancer cells evade immune recognition. By inhibiting this pathway, the medicine is designed to support the immune system’s ability to recognize and attack cancer cells. The new recommendation expands the medicine’s existing Category 2A designation for metastatic cutaneous squamous cell carcinoma, which was assigned in 2025. The U.S. Food and Drug Administration (FDA) approved UNLOXCYT in 2024 for adults with metastatic or locally advanced cutaneous squamous cell carcinoma who are not candidates for curative surgery or radiation. The updated guideline recommendation may help inform treatment decisions and access pathways for eligible patients. The NCCN designation is a clinical guideline recommendation, not a new FDA approval. It reflects the evidence reviewed by the guideline panel for the specified patient population.
NCCN Guidelines Recognize UNLOXCYT for Locally Advanced Skin Cancer
Cutaneous squamous cell carcinoma (CSCC) is a common form of skin cancer that develops from squamous cells in the skin’s outer layers. While many cases can be treated successfully when detected early, advanced disease can invade surrounding tissues and structures, including nerves and blood vessels. Locally advanced CSCC may become difficult to manage when surgery or radiation cannot provide a curative option. For these patients, systemic therapies that target cancer biology or activate immune responses can be important treatment considerations. The updated NCCN guidelines now identify UNLOXCYT as a Category 2A preferred option for eligible patients with locally advanced disease who cannot undergo curative surgery or radiation. Category 2A recommendations generally indicate that there is uniform NCCN consensus that an intervention is appropriate, based on lower-level evidence. The recommendation is intended to help healthcare professionals evaluate treatment options within the context of each patient’s condition, medical history and treatment goals. UNLOXCYT’s existing recommendation for metastatic CSCC remains an important part of its clinical positioning. The expanded designation reflects growing recognition of the clinical evidence supporting its use in advanced disease. However, treatment selection must still account for individual circumstances, potential adverse effects and applicable prescribing information.
Clinical Study Reports Durable Responses With Cosibelimab
The updated recommendation is supported by findings from the pivotal registration study evaluating cosibelimab in advanced CSCC. In the locally advanced patient group, 55% of patients achieved an objective response, including 26% who experienced a complete response, according to the data reported by Sun Pharma. An objective response includes either a complete response, in which detectable cancer disappears according to the study’s criteria, or a partial response, in which the tumor decreases by a specified amount. At a median follow-up of 24.1 months, the median duration of response had not been reached, indicating that the available follow-up was insufficient to determine the point by which half of the responding patients had lost their response. The reported duration-of-response range extended from 8.3 months to more than 31.3 months. Sun Pharma also reported that fewer than 1% of patients experienced high-grade, Grade 3–5 immune-mediated adverse reactions in the cited analysis, with no Grade 4 or Grade 5 immune-mediated reactions reported. These findings support continued evaluation of the treatment’s durability and safety profile. Nevertheless, immunotherapies can cause serious or potentially fatal immune-related complications affecting organs such as the lungs, liver, intestines, kidneys and endocrine glands. Patients receiving UNLOXCYT require appropriate monitoring and management by qualified healthcare professionals.
Sun Pharma Advances Immuno-Oncology Treatment Options
UNLOXCYT is a PD-L1-blocking monoclonal antibody developed for adults with advanced CSCC who are not candidates for curative surgery or radiation. By targeting PD-L1, the medicine interferes with an immune checkpoint pathway that tumors may use to suppress antitumor immune responses. Sun Pharma’s broader portfolio includes specialty medicines, generics and innovative treatments across several therapeutic areas, including dermatology and oncology. The updated NCCN recommendation strengthens the clinical position of UNLOXCYT for eligible patients with locally advanced disease and may influence institutional treatment pathways, payer coverage assessments and prior-authorization decisions. However, guideline inclusion does not guarantee access or reimbursement in every healthcare setting. The medicine’s prescribing information continues to outline important risks, including severe immune-mediated adverse reactions, infusion-related reactions and potential fetal harm. Healthcare professionals must weigh these risks against potential benefits when selecting treatment. The expanded NCCN recommendation marks a significant milestone for UNLOXCYT in advanced skin cancer care, while ongoing clinical experience will continue to inform its role in oncology practice.
