NEW YORK, March 19, 2026
Pfizer Inc. announced that its Phase 3 TALAPRO-3 clinical trial evaluating TALZENNA® (talazoparib) in combination with XTANDI® (enzalutamide) has met its primary endpoint, demonstrating a statistically significant and clinically meaningful improvement in radiographic progression-free survival (rPFS) in patients with HRR gene-mutated metastatic hormone-sensitive prostate cancer (mHSPC). The results highlight a substantial reduction in the risk of disease progression or death, reinforcing the potential of this combination as a next-generation precision oncology therapy.
Phase 3 Trial Demonstrates Significant rPFS Benefit
The TALAPRO-3 study, a global, randomized, double-blind, placebo-controlled Phase 3 trial, enrolled 599 patients across multiple regions, including North America, Europe, and Asia-Pacific. The study demonstrated that TALZENNA combined with XTANDI significantly extended rPFS compared to placebo plus XTANDI, exceeding the pre-specified efficacy threshold and confirming robust clinical benefit.
Importantly, the efficacy benefit was consistent across patients with both BRCA and non-BRCA homologous recombination repair (HRR) gene mutations, indicating broad applicability of the treatment across genetically defined patient populations. A majority of patients receiving the combination therapy remained progression-free at the time of analysis, reflecting strong disease control in a high-risk population.
Additional improvements were observed in secondary endpoints, including objective response rate (ORR), duration of response (DOR), and time to prostate-specific antigen (PSA) progression, further strengthening the clinical significance of the findings.
Encouraging Survival Trends and Established Safety Profile
An interim analysis revealed a strong trend toward improved overall survival (OS), a key endpoint in oncology trials, although the data are still maturing. This early signal suggests that the combination therapy may offer long-term survival benefits in addition to delaying disease progression.
The safety profile of TALZENNA plus XTANDI was consistent with previously established profiles for each therapy, with no new safety signals identified. Common adverse events included hematologic toxicities such as anemia, neutropenia, and thrombocytopenia, which were manageable with appropriate clinical monitoring and supportive care.
These findings support the potential use of the combination therapy in earlier stages of prostate cancer, expanding its role beyond current indications and providing new treatment options for patients with limited alternatives.
Advancing Precision Oncology in Prostate Cancer
Prostate cancer remains one of the most common cancers worldwide, with a significant proportion of patients progressing to advanced stages despite existing therapies. Approximately 25% of metastatic prostate cancer cases harbor HRR gene mutations, which are associated with poorer prognosis and resistance to standard treatments.
TALZENNA, a PARP inhibitor, works by blocking DNA damage repair mechanisms in cancer cells, while XTANDI, an androgen receptor pathway inhibitor, suppresses tumor growth signals. Together, this combination delivers a dual mechanism approach, targeting both genomic instability and hormonal pathways, offering a highly effective precision medicine strategy.
Pfizer plans to engage with global regulatory authorities to explore potential label expansion of TALZENNA in earlier disease settings, reflecting confidence in the strength of the clinical data and its potential impact on treatment paradigms.
Transforming the Treatment Landscape for Advanced Cancer
The TALAPRO-3 results represent a significant advancement in the treatment of metastatic prostate cancer, particularly for patients with HRR gene mutations who face high risk of disease progression. By demonstrating clinically meaningful improvements in progression-free survival and promising survival trends, the combination of TALZENNA and XTANDI has the potential to reshape the standard of care in oncology.
This development underscores the importance of biomarker-driven therapies, rigorous clinical research, and adherence to GxP standards, ensuring that innovative treatments meet the highest standards of safety, efficacy, and regulatory compliance before reaching patients.
