Insulet Corporation has announced the enrollment of the first patient in its EVOLVE pivotal clinical study, evaluating a fully closed-loop automated insulin delivery (AID) system for people with type 2 diabetes (T2D). This milestone marks a significant advancement in medical device innovation and diabetes management, as the company aims to eliminate the need for manual insulin dosing and redefine patient care through algorithm-driven automation.
Insulet Advances Fully Closed-Loop Diabetes Technology
The EVOLVE study is designed as a multi-center randomized controlled trial, enrolling up to 350 adult participants across 40 U.S. clinical sites, to evaluate the safety and efficacy of Insulet’s next-generation AID system compared to standard insulin therapy. This investigational system leverages a novel adaptive algorithm trained on real-world and simulated patient data to automatically adjust insulin delivery without user intervention, including eliminating the need for mealtime bolusing or manual input.
This innovation represents a major shift in diabetes care, particularly for patients with type 2 diabetes who require insulin therapy but often struggle with complex dosing regimens. By removing manual steps, the system is expected to significantly reduce patient burden and improve adherence, while also simplifying workflows for healthcare providers.
Clinical Design and Regulatory Progress
The EVOLVE pivotal study follows a series of feasibility trials and is supported by an Investigational Device Exemption (IDE) approval from the U.S. FDA, granted in March 2026. The study will assess outcomes such as glycemic control, safety, and usability, providing critical data to support a planned 510(k) regulatory submission in 2027.
Previous feasibility data presented at international scientific conferences demonstrated promising outcomes, including a 24% improvement in time-in-range (TIR) compared to standard injection therapy, highlighting the potential of the system to deliver clinically meaningful improvements in glucose management. Additionally, earlier pivotal trials with Insulet’s Omnipod® platform showed reductions in HbA1c levels from 8.2% to 7.4% and a 20% increase in TIR, further supporting the effectiveness of automated insulin delivery technologies.
Transforming Diabetes Care and Expanding Access
Insulet’s fully closed-loop system is designed to address a critical gap in diabetes care, as less than 5% of insulin-dependent type 2 diabetes patients currently use automated insulin delivery systems, despite strong recommendations from clinical guidelines. The company aims to expand adoption, particularly in primary care settings, where the majority of diabetes patients are managed.
By eliminating the need for extensive training, manual setup, and continuous adjustments, the system is expected to increase accessibility and scalability of advanced diabetes technologies. This approach aligns with broader healthcare goals of improving patient outcomes, reducing clinical workload, and enabling precision-driven treatment strategies.
The company plans to launch the system commercially by 2028, pending regulatory approval, positioning itself at the forefront of next-generation diabetes technology innovation.
Impact on MedTech and Digital Health Innovation
The EVOLVE study underscores the growing convergence of medical devices, digital health, and artificial intelligence in chronic disease management. Fully closed-loop systems represent the next frontier in diabetes care, moving toward autonomous, data-driven treatment models that can continuously adapt to patient needs in real time.
This advancement is expected to drive significant growth in the MedTech sector, particularly in wearable and connected devices that integrate continuous glucose monitoring (CGM) with automated insulin delivery systems. By combining clinical validation, regulatory strategy, and advanced algorithmic design, Insulet is setting a new benchmark for innovation in diabetes care.
