SOUTH SAN FRANCISCO, California, October 15, 2026
insitro announced a worldwide exclusive research collaboration with AstraZeneca to discover novel therapeutic targets and medicines for metabolic dysfunction-associated steatohepatitis (MASH) and cardiomyopathies using artificial intelligence (AI) and causal human biology. Under the agreement, insitro will receive $40 million upfront and is eligible to receive up to approximately $1.46 billion in potential discovery, development and commercial milestone payments, in addition to tiered, single-digit royalties on potential global commercial sales. The collaboration will apply insitro’s Virtual Human™ platform to identify causal disease biology across both indications and advance AstraZeneca-nominated targets from discovery through development candidate selection. AstraZeneca will lead subsequent clinical development, regulatory activities and commercialization. The partnership focuses on diseases characterized by substantial patient heterogeneity, where individuals sharing a diagnosis may have different underlying biological mechanisms. By applying AI to complex human and cellular datasets, the companies aim to identify disease drivers and support the discovery of new therapeutic approaches. The agreement combines insitro’s computational and biological discovery capabilities with AstraZeneca’s pharmaceutical development expertise to pursue potential treatments for conditions associated with significant unmet medical needs.
Virtual Human Platform Targets Disease Biology
insitro will use its Virtual Human™ causal AI discovery platform to investigate the biological mechanisms underlying MASH and cardiomyopathies. Both disease areas present challenges because patients grouped under the same diagnosis may differ in the biological pathways driving their conditions. The collaboration is designed to address this heterogeneity by analyzing high-content datasets to identify causal disease drivers and potential therapeutic targets. According to insitro, its platform integrates human and cellular data with machine learning to reveal how diseases begin, progress and may be resolved. The company’s approach aims to distinguish causal biological mechanisms from associations that may not directly drive disease pathology. insitro Founder and Chief Executive Officer Daphne Koller said the collaboration would combine the company’s ability to investigate complex biological datasets with AstraZeneca’s pharmaceutical expertise and development scale. The agreement also reflects a growing role for AI-enabled research in therapeutic discovery, particularly in disease areas where identifying the appropriate biological target is a critical step toward developing effective medicines. The targets identified through the collaboration will be nominated by AstraZeneca, with insitro responsible for advancing the programs through the discovery stage to development candidate.
AstraZeneca to Lead Clinical Development
Under the collaboration, AstraZeneca will lead clinical development, regulatory work and commercialization for programs emerging from the research agreement. insitro will focus on target discovery and advancement of AstraZeneca-nominated targets through the development-candidate stage. The financial terms include the $40 million upfront payment, potential milestone payments of approximately $1.46 billion across discovery, development and commercial achievements, and tiered single-digit royalties based on global commercial sales. The milestone amount represents potential payments tied to future program progress and commercial outcomes rather than guaranteed revenue. Sharon Barr, Executive Vice President and Head of BioPharmaceuticals Research and Development at AstraZeneca, said the collaboration supports the company’s strategy of applying AI to identify biological drivers of disease and improve target selection. Both companies expect the partnership to bring together data-driven biological discovery and pharmaceutical development capabilities to pursue new medicines. The announcement does not identify specific targets or individual drug candidates arising from the collaboration, and further research will be required to establish whether the programs can progress into clinical testing and demonstrate therapeutic benefit..
insitro Expands Its AI-Driven Therapeutic Discovery Strategy
Founded in 2018 and led by Daphne Koller, insitro combines large-scale human and cellular datasets, machine learning and experimental biology to discover medicines directed at causal disease mechanisms. Its Virtual Human™ platform is designed to identify genetic and biological drivers of disease, while its TherML™ platform focuses on designing medicines against targets identified through the discovery process. The company says it is advancing a pipeline of nine programs, including genetic regulators of liver fat and fibrosis expected to enter clinical development in early 2027, an oligonucleotide therapy being investigated for amyotrophic lateral sclerosis (ALS), and a genetic regulator of brown fat associated with metabolic health research. insitro also reports collaborations with Bristol Myers Squibb, Eli Lilly and Gilead, alongside financial backing from investors including Andreessen Horowitz, ARCH, BlackRock, GV, T. Rowe Price and Temasek. The new agreement adds MASH and cardiomyopathies to the company’s partnered discovery activities. For AstraZeneca, the collaboration provides access to insitro’s AI-enabled approach to target identification in complex diseases. The programs’ progress will depend on the biological findings, subsequent candidate development and clinical evidence generated through the partnership.
