Guardant Health announced that its Shield blood test for colorectal cancer (CRC) screening will receive coverage through Carelon Health Services, expanding access to the FDA-approved blood-based screening option for eligible policyholders. The updated coverage policy is scheduled to become effective November 15, 2026, representing another important step in Guardant Health’s efforts to expand the availability of non-invasive colorectal cancer screening across the United States. With the Carelon policy update, approximately 90 million people ages 45 and older are expected to have coverage for Shield, representing about 75% of eligible U.S. patients for CRC screening. The coverage expansion strengthens the commercial reach of Shield and supports Guardant Health’s strategy of making blood-based cancer screening more accessible through routine healthcare visits.
Shield Coverage Expands Across U.S. Population
The Carelon decision adds another major coverage milestone for Guardant Health’s Shield test, which is designed to provide an alternative to more invasive or logistically challenging screening approaches. Carelon Health Services provides medical benefit management services for Blue Cross Blue Shield plans across 26 states, extending the potential reach of the Shield test across a broad U.S. patient population. Guardant Health said the latest coverage expansion increases the number of people with access to Shield to approximately 90 million, bringing the test within reach of the majority of Americans who meet the age and risk criteria for average-risk CRC screening. The development is particularly relevant as colorectal cancer screening remains a major preventive healthcare priority and screening participation can be affected by barriers associated with conventional testing methods. Shield is intended for average-risk adults aged 45 and older and can be completed through a blood draw during a routine healthcare visit.
Blood-Based CRC Screening Gains Momentum
Shield is positioned by Guardant Health as a non-invasive blood-based colorectal cancer screening test that uses methylation partitioning cell-free DNA technology to detect alterations associated with CRC in blood. The test is the first FDA-approved blood test for primary colorectal cancer screening in average-risk adults aged 45 and older. Guardant Health said more than 200,000 Shield tests have been performed in real-world settings, with studies demonstrating increased screening completion among patients due for CRC screening. The company also reported screening adherence of more than 90% across practice and health system settings. Shield has been recommended by the American Cancer Society and National Comprehensive Cancer Network as a non-invasive screening option. Importantly, a positive Shield result does not establish a cancer diagnosis and should be followed by colonoscopy evaluation to investigate the possibility of colorectal cancer or advanced adenoma.

The expanded Carelon coverage strengthens the commercial position of Shield within the U.S. colorectal cancer screening market and adds to Guardant Health’s broader precision oncology strategy.
