Creyos has announced initial results from its implementation of digital cognitive testing across Claremedica, a value-based senior primary care organization serving more than 34,000 Medicare Advantage patients across over 35 sites in Florida. During the first seven months after replacing paper-based cognitive screening with Creyos’ digital assessment platform, Claremedica reported that more than 90% of newly identified dementia cases were detected at an early or mild stage, when proactive care planning may still be possible. The implementation also reduced average screening time from approximately 12 minutes to six minutes, highlighting the potential of digital cognitive assessment to improve the efficiency and timing of cognitive screening in primary care.
Digital Testing Identifies Earlier Cognitive Impairment
Claremedica previously used the Mini-Mental State Examination (MMSE) for organization-wide cognitive screening for more than two years. According to the results shared by Creyos, nearly 80% of patients newly identified with dementia had previously been screened by Claremedica using the MMSE, in some cases more than once, without their cognitive decline registering on the earlier assessment. Following the transition to Creyos, Claremedica’s dementia identification rate more than doubled, with the company attributing the increase primarily to the identification of earlier-stage cases rather than changes in diagnostic practices.
The results highlight the potential importance of more sensitive approaches to cognitive assessment in primary care, where early cognitive changes can be difficult to recognize through traditional screening methods. Creyos replaces a single pass-or-fail screening score with a domain-level cognitive profile covering areas such as memory, attention, reasoning, and executive function. The assessment can be administered by a medical assistant on a tablet without requiring direct physician supervision, while results are returned as structured data with recommended next steps.
AI-Enabled Assessment Improves Screening Efficiency
The implementation also produced improvements in screening efficiency. Creyos reported that average cognitive screening time fell from approximately 12 minutes with the MMSE to about six minutes using its digital assessment, potentially returning nearly 90 minutes per day to a typical physician’s caseload. The platform supports 13 languages, including Haitian Creole, which Claremedica identified as important for its patient population.
Creyos also reports integration with more than 90 electronic health record systems, including Epic and athenahealth, supporting incorporation into existing clinical workflows. Claremedica reported strong adoption following implementation, with more than 95% adoption on the first day. The organization attributes the successful rollout in part to physician engagement and education about the scientific evidence supporting the digital cognitive assessment platform. Creyos’ co-founder and Chief Scientific Officer, Professor Adrian Owen, presented research supporting the platform to Claremedica clinicians before patient screening began. The company said the approach was intended to provide clinicians with evidence regarding the validation and development of the underlying cognitive assessment tasks.
Claremedica Tracks Long-Term Patient Outcomes
The reported results come as healthcare providers increasingly explore digital approaches to cognitive health assessment and dementia detection. Creyos describes its platform as an FDA Class II-registered clinical decision-support tool combining validated online cognitive tasks with digitized behavioral health questionnaires. Claremedica is now tracking whether earlier identification of cognitive impairment is associated with downstream outcomes over the next 12 to 24 months. These measures include falls, avoidable hospitalizations, and total cost of care, which could provide additional insight into the potential clinical and operational impact of earlier dementia identification.
The organization’s experience with Creyos demonstrates how digital cognitive assessment can be integrated into large-scale primary care workflows while reducing screening time and generating more detailed cognitive profiles. However, the reported findings are initial implementation results, and the longer-term outcomes being tracked by Claremedica will be important for assessing whether earlier identification translates into measurable improvements in patient care and healthcare utilization. The adoption of digital cognitive testing represents a growing application of healthcare technology and data-driven clinical decision support in senior care.
