Novo Nordisk has reported compelling Phase 3 results demonstrating that CagriSema 2.4 mg/2.4 mg achieved significant reductions in both HbA1c and body weight across multiple studies in adults with type 2 diabetes, reinforcing the potential of the investigational therapy as a next-generation treatment for diabetes and metabolic disease. The findings were presented during a late-breaking symposium at the American Diabetes Association (ADA) 2026 Scientific Sessions in New Orleans and simultaneously published in leading medical journals. CagriSema combines cagrilintide, a long-acting amylin receptor agonist, with semaglutide, a GLP-1 receptor agonist, creating a dual-mechanism approach designed to address blood glucose control and weight management simultaneously. The REIMAGINE clinical development program evaluated the once-weekly therapy across multiple patient populations and disease stages, producing consistently positive outcomes that could position CagriSema as a first-in-class amylin and GLP-1 combination therapy for type 2 diabetes.
REIMAGINE Trials Deliver Strong HbA1c and Weight-Loss Outcomes
Across the REIMAGINE Phase 3 program, CagriSema consistently met its primary endpoint of HbA1c reduction while also achieving confirmatory secondary endpoints related to body weight reduction. In REIMAGINE 1, adults with type 2 diabetes inadequately controlled through diet and exercise achieved a 1.8% reduction in HbA1c and a 13.8% reduction in body weight with CagriSema 2.4 mg/2.4 mg over 40 weeks, significantly outperforming placebo. In the larger REIMAGINE 2 trial, which enrolled more than 2,700 participants, CagriSema demonstrated superiority over semaglutide and cagrilintide monotherapies.
Patients receiving the highest dose achieved a 1.91% reduction in HbA1c and a 14.2% reduction in body weight, highlighting the therapeutic advantage of combining amylin and GLP-1 pathways. Researchers noted that these results underscore the potential of dual-hormone therapy to improve glycemic control while delivering clinically meaningful weight reduction, a critical objective in the treatment of type 2 diabetes where obesity often contributes to disease progression and cardiovascular risk.
Novel Amylin and GLP-1 Combination Expands Treatment Potential
The scientific rationale behind CagriSema centers on combining two complementary metabolic hormones. Amylin, naturally co-secreted with insulin following food intake, plays a key role in appetite regulation, satiety, glycemic control, and body weight management. By pairing cagrilintide with semaglutide, Novo Nordisk has developed a treatment designed to address multiple physiological pathways involved in diabetes and obesity. Experts involved in the REIMAGINE program emphasized that the combination could provide meaningful benefits for patients who require stronger metabolic control than existing therapies can offer.
The data presented at ADA 2026 demonstrated that these benefits remained consistent across individuals at different stages of disease progression, supporting the broad applicability of the therapy. The results further strengthen growing scientific interest in amylin-based treatments as an important complement to GLP-1 receptor agonists in future diabetes management strategies.
Benefits Extend to Insulin-Treated Type 2 Diabetes Patients
Additional evidence from REIMAGINE 3 highlighted the therapy’s effectiveness among adults with type 2 diabetes receiving basal insulin therapy. Participants treated with CagriSema 2.4 mg/2.4 mg achieved a 2.33% reduction in HbA1c and a 12.0% reduction in body weight, significantly outperforming placebo while maintaining a safety profile generally consistent with incretin-based therapies. Gastrointestinal adverse events were the most commonly reported side effects across the clinical program, with relatively low discontinuation rates considering the magnitude of efficacy observed.
Novo Nordisk believes these findings support the continued advancement of CagriSema as a transformative therapy capable of addressing two of the most pressing challenges in diabetes care—blood glucose control and obesity. With regulatory review already underway for weight management indications and additional studies continuing, CagriSema is emerging as one of the most closely watched metabolic disease therapies in development, potentially offering a new standard of care for millions of people living with type 2 diabetes worldwide.
