BioLineRx Ltd. and Hemispherian AS have announced encouraging new preclinical data demonstrating a strong synergistic anti-tumor effect when their investigational small-molecule therapy GLIX1 is combined with the PARP inhibitor olaparib in a patient-derived ovarian cancer xenograft (PDX) model. The findings provide compelling evidence supporting synthetic lethality between GLIX1 and PARP inhibitors, suggesting the combination may significantly expand the therapeutic potential of PARP inhibition in homologous recombination (HR)-proficient ovarian cancer, where currently available PARP inhibitors have limited clinical benefit. The low-dose GLIX1 and olaparib combination demonstrated substantially greater anti-tumor activity than either therapy alone, while achieving efficacy comparable to cisplatin, one of the current standards of care for ovarian cancer. The companies believe these results strengthen the scientific rationale for incorporating ovarian cancer into the expansion phase of the ongoing Phase 1/2a clinical trial evaluating GLIX1 and represent an important step toward developing new treatment options for patients with difficult-to-treat ovarian cancers.
Preclinical Study Demonstrates Strong Synergy with PARP Inhibition
The newly reported research evaluated GLIX1 in combination with the PARP inhibitor olaparib using a clinically relevant patient-derived xenograft model of HR-proficient ovarian cancer. The study included six treatment groups comprising cisplatin, GLIX1 monotherapy, olaparib monotherapy, low-dose GLIX1, a low-dose GLIX1-olaparib combination, and a control arm. Results demonstrated that the combination therapy significantly outperformed both monotherapy treatments and the untreated control despite utilizing lower doses of each investigational agent.
Remarkably, tumor reduction achieved with the low-dose combination was comparable to that observed with cisplatin chemotherapy, highlighting the potential of GLIX1 to improve treatment outcomes while potentially reducing toxicity associated with higher-dose therapies. The findings build upon earlier in vitro studies, which consistently demonstrated strong synergy between GLIX1 and multiple PARP inhibitors across several HR-proficient ovarian cancer cell lines. According to the companies, GLIX1 activates the TET2 pathway, inducing DNA damage that overwhelms cancer cell repair mechanisms and creates the conditions necessary for synthetic lethality, enabling PARP inhibitors to become effective even in tumors that are traditionally resistant to this class of therapy.
Clinical Development Expands Toward New Ovarian Cancer Indications
Ovarian cancer remains one of the deadliest gynecologic malignancies worldwide, with patients who have HR-proficient tumors or platinum-resistant disease facing limited therapeutic options and poorer clinical outcomes. Current PARP inhibitors primarily benefit patients with BRCA-mutated or HR-deficient cancers, leaving a substantial unmet medical need for therapies capable of extending these benefits to a broader patient population. Based on the encouraging preclinical findings, BioLineRx and Hemispherian plan to include an ovarian cancer cohort in the expansion portion of the ongoing Phase 1/2a clinical study evaluating GLIX1, which is currently enrolling patients with recurrent glioblastoma and other high-grade gliomas. GLIX1 is a first-in-class, orally administered, brain-penetrant small molecule designed to activate the TET2 pathway and induce apoptotic cancer cell death through targeted DNA damage mechanisms.
The companies also intend to present these promising preclinical data at upcoming international medical conferences as they continue advancing clinical development across multiple solid tumor indications. If future clinical studies confirm these findings, the GLIX1-PARP inhibitor combination could significantly broaden the clinical use of PARP inhibitors, improve treatment outcomes for HR-proficient ovarian cancer patients, and provide an innovative targeted therapeutic strategy for individuals with limited treatment options.
