Alzheon, Inc. announced that it will present nine scientific posters at the 2026 Alzheimer’s Association International Conference (AAIC) in London, highlighting new analyses from its Phase 3 APOLLOE4 study and Phase 2 biomarker trials evaluating valiltramiprosate (ALZ-801), an investigational oral therapy for Alzheimer’s disease. The presentations will focus on patients carrying two copies of the APOE4 gene, the population at highest genetic risk for Alzheimer’s disease and particularly vulnerable to complications associated with anti-amyloid antibody therapies. The company will present new evidence demonstrating favorable long-term safety, low rates of amyloid-related imaging abnormalities (ARIA), neurovascular protection, advanced brain imaging findings, plasma biomarker improvements, and sustained clinical benefits. Collectively, the data strengthen Alzheon’s precision medicine strategy and support the continued development of valiltramiprosate as a potential first-in-class oral disease-modifying therapy for Alzheimer’s disease.
Phase 3 Analyses Demonstrate Neurovascular Protection and Sustained Clinical Benefits
The AAIC presentations include expanded analyses from the placebo-controlled Phase 3 APOLLOE4 trial and its long-term extension, together with findings from Phase 2 biomarker studies. Although the original Phase 3 trial did not achieve its primary endpoint, the company reported encouraging evidence demonstrating clinically meaningful benefits among patients with mild cognitive impairment (MCI) carrying the APOE4/4 genotype. New analyses showed lower rates of ARIA, including brain edema and microhemorrhage, compared with placebo, supporting the therapy’s favorable neurovascular safety profile in this high-risk population. Long-term extension studies extending treatment for up to four years also demonstrated sustained cognitive stability, slower brain atrophy, preservation of hippocampal microstructure, and continued absence of symptomatic ARIA-E or ARIA-H. Additional plasma biomarker analyses reported reductions in p-tau217, one of the leading biomarkers associated with Alzheimer’s pathology, further supporting the potential disease-modifying activity of valiltramiprosate.
Advanced Imaging and Biomarker Research Strengthen Precision Medicine Strategy
Several presentations will highlight sophisticated imaging and biomarker analyses designed to better understand disease progression and treatment response. Researchers will present diffusion tensor imaging (DTI), volumetric MRI (vMRI), cerebellar volume assessments, and biomarker correlations demonstrating preservation of brain structure alongside improved clinical outcomes. The conference will also feature quantitative systems pharmacology (QSP) modeling evaluating valiltramiprosate as a potential oral maintenance therapy following treatment with anti-amyloid monoclonal antibodies such as donanemab and lecanemab. According to Alzheon, these analyses reinforce the scientific rationale for combining precision genetics, plasma biomarkers, and advanced neuroimaging to identify patients most likely to benefit from therapy while minimizing treatment-related risks.
Oral Alzheimer’s Therapy Continues Late-Stage Clinical Development
Valiltramiprosate (ALZ-801) is a Phase 3 investigational oral small-molecule therapy designed to inhibit the formation of toxic soluble beta-amyloid oligomers that contribute to Alzheimer’s disease progression. Unlike anti-amyloid antibody therapies that remove existing amyloid plaques, valiltramiprosate acts upstream by preventing the formation of neurotoxic oligomers while maintaining a favorable safety profile. The drug previously received FDA Fast Track designation and is being developed initially for Alzheimer’s patients carrying the APOE4/4 genotype, representing approximately 15% of all Alzheimer’s patients, with future plans to expand into broader APOE4 populations. Through its precision medicine platform integrating genetics, biomarkers, and targeted therapeutics, Alzheon aims to deliver safer, personalized treatment options for patients living with Alzheimer’s disease and other neurodegenerative disorders.
